Anyone in need of stable housing solutions or requiring home health services is eligible for referral to our Housing Stabilization Service (HSS) and home health agency. Whether you are seeking assistance for yourself or know someone who could benefit from our support, we welcome referrals from individuals, families, caregivers, healthcare professionals, social workers, and community organizations. Our dedicated team is here to provide compassionate assistance and support to all those who require it.
